Are the above grid just for Twitter bootstrap or also for Skeleton ? Any suggestions how to also plan for screens that are wider than usual (such as large apple monitors) ?

@Julie this is only for Bootstrap. The "Large" desktop template above is for min-width: 1200px fixed width layout. You could use that for larger layouts.

Do I need to resize elements again and again for all devices. For example, let's we say I've created my default wireframe (1024x768) for desktop and When I try to place them into grid layout for smart phone Do I have to resize them? And why there is no column in the smartphone wireframe? There is 12 column for desktop and tablets? I confused please help me .)

Here is some more information on the Bootstrap grid: http://getbootstrap.com/css/#grid. The reason there is no columns in the smartphone wireframe is because the Bootstrap grid has no columns for the smartphone size. Hopefully this helps a bit!

Can this be used for standard responsive design mockup i.e. HTML, Wordpress, et.c?

Some HTML is allowed. You can also use Mockups text formatting markup.
Ex: *bold* _italic_ &underlined& {color:#FF0000}colored{color} * New lines with asterisks for bulleted lists.
